Removing a spouse: Spouses can only be removed through divorce or death.
Complete a new Personal Choice application and enclose a copy of the divorce decree or death certificate.
(remove divorced spouses immediately or you will be financially responsible for their medical claims)
ONCE YOU ARE DIVORCED, YOUR DIVORCED SPOUSE IS INELIGIBLE ON YOUR LEHB MEDICAL, DENTAL, PRESCRIPTION AND VISION COVERAGE.
Any medical, dental, prescription and/or vision benefits incurred by your ex-spouse after the date of divorce are ineligible and will be retracted by LEHB.
You and your ex-spouse will be financially responsible for claims incurred after the date of divorce.
Please ensure, in you divorce decree, your ex-spouse's medical coverage is addressed either through your ex-spouse's employer or COBRA coverage through LEHB.
LEHB will not, UNDER ANY CIRCUMSTANCES, use LEHB members' money to pay claims for an ineligible member.
