L.E.H.B. PRESCRIPTION FUND

IMPORTANT PRESCRIPTION BENEFIT CHANGES - APRIL 1, 2006

All L.E.H.B. proposed changes have been overwhelmingly approved by the membership completing surveys at the 22 city-wide L.E.H.B. meetings.  Members approved all L.E.H.B. proposals addressing the arbitration panel $700,000 “cost-shifting” to members via plan benefit design change.

L.E.H.B. Prescription Fund will remain mandatory generic (if you select a name brand when generic is available you will be financially responsible for difference in cost.)

CHANGES EFFECTIVE APRIL 1, 2006 COPAYS:

Retail pharmacy - Prescriptions at a network pharmacy up to a 30 day supply:

  • Generic Drug (Chemical copies of their brand name)
  • Brand Name Drug (Trade name under which the product is advertised and sold) Patent protected for 17 years
  • Mail Order – Pre-$2.00 copayscriptions filled through ESI Mail Order up to a 90 day Supply

  • Generic Drug (Chemical copies of their brand name)
  • Brand Name Drug (Trade name under which the product is advertised and sold) Patent protected for 17 years
  • You should receive medication within 14 days

  • CURASCRIPT (Specialty Injectables)

  • CuraScript has been chosen as your preferred specialty pharmacy.
  • CuraScript specializes in oral and injectable specialty medications.  Specialty Products are those biotech or injectable prescription drug products requiring special handling.
  • CuraScript offers many products and services.
  • Medications that are processed through CuraScript are limited to a 30 day supply and have the following copayments:

    OVER THE COUNTER (OTC) CLARITIN AND PRILOSEC
    Over the counter (OTC) means medications that are available to you without a prescription.  In the past couple of years two medications went to OTC status and no longer require a prescription.  These medications are Prilosec (20 mg strength only) and Claritin (all strengths).  L.E.H.B. OTC limited to these two prescriptions.
