New Baby: Send in a copy of the birth certificate listing both the child and member's names with a new Personal Choice Application adding the baby.  Children awaiting adoption are eligible for coverage once they have a docket number. (Effective date is date of birth if the baby is added within two months of birth or the baby will be added the first of the month you provide us with the information.)
