Just Married: Send in a copy of the marriage certificate with a new Personal Choice Application adding the spouse (effective date for spouse is the first of the next month)

When you get married: You must fill out an application listing the name, birth date, and social security number of your spouse with a copy of the marriage license. Blue Cross will not enroll a spouse back dated more than two months.
