your credit card!

Your LEHB premium is based on total dollars used for medical services.
Medical services that clearly were caused from an "on the job incident"
should not be paid by your LEHB Personal Choice Fund.

FACTS

Þ You MUST notify your supervisor immediately.

Þ You MUST follow Police Department guidelines and complete an employee injury report within forty-eight (48) hours.

Þ You MUST be notified in writing of any City denial.

Þ You MUST appeal the City denial within fifteen (15) days. Civil Service 32.1211
