¨ Who can benefit from this program?

Any member who has been diagnosed with any of the following:

  • Asthma
  • Childhood Asthma
  • Diabetes
  • Total Joint Replacement
  • Congestive Heart Failure
  • Heart Attacks
  • By-Pass Surgery
  • Angioplasty
  • Expectant Mothers—assist in preparing for the birth
  • HIV/Aids

¨ How do I benefit by using the Connection Program?

  • You will be contacted by a nurse case manager by phone from Independence Blue Cross
  • Will ensure enrolled members receive all the medical treatment they require as ordered by the physician.
  • Ensure the member is educated about his/her medical and prescription treatment
  • Will assist with all medical equipment ordered by the physician.

¨ Does Connection Program cost anything?

No.

¨ How do I enroll in a Connection Program?
