Dear LEHB Member:
Please use the below procedures to expedite the Blue Cross
payment for medical services provided when involved in an automobile
accident.
- You must have the Pennsylvania State minimum medical insurance
coverage of at least five thousand dollars ($5,000.00), with
your automobile insurance carrier.
- It is the responsibility of your attorney or yourself
to submit your medical claims first to your auto insurance carrier,
and secondly to Blue Cross. Your attorney or yourself must mail
the below listed information to Independence Blue Cross, Attn;
Subrogation Department, 1901 Market Street, 19th Floor,
Philadelphia, PA 19103:
- Copy of your automobile insurance
policy, stating your medical dollar limit that your auto policy
is responsible for.
- A summary of the medical
bills, adding up to that dollar limit with proof of payment by
your automobile insurance carrier.
- A summary of the medical
bills, adding up to that dollar limit with proof of payment by
your automobile insurance carrier.
- The itemized medical bills
which support the above summary.
- The itemized bills over your
automobile insurance dollar limit, with a breakdown of Blue Cross
claims (hospital) Blue Shield claims (doctor) and Major Medical
or Personal Choice.
- When these bills are submitted,
please ask Blue Cross to process under secondary coverage. Note;
make sure you or your attorney has a copy of all bills, submitted
to your auto insurance carrier and Blue Cross.
